ABSTRACT

Resumen Objetivo: analizar las características epidemiológicas, bases etiopatogénicas y presentación clínica, así como el diagnóstico y el tratamiento de la hiperplasia de glándulas de Brunner (HGB). Métodos: describir un caso de HGB diagnosticado de forma incidental durante una endoscopia electiva y realizar una revisión de la literatura disponible hasta el momento. Resultados: esta neoformación consiste en una proliferación glandular localizada preferentemente en el duodeno proximal. Su diagnóstico, normalmente realizado mediante biopsia endoscópica, puede asociarse con complicaciones que, aunque infrecuentes, no deben ser subestimadas. Conclusiones: las neoplasias duodenales representan un porcentaje pequeño dentro del total de las que afectan al tracto gastrointestinal. Debido a que el diagnóstico de estas lesiones suele realizarse de forma casual durante una endoscopia programada, el tratamiento deberá basarse en la sintomatología, así como el tamaño de las mismas, de acuerdo con los estándares de tratamiento de cada centro.


Abstract Objective: This study analyzes the epidemiological characteristics, etiological and pathogenic bases, clinical presentation, diagnosis and treatment of Brunner's gland hyperplasia. Methods: We describe a case of Brunner's gland hyperplasia that was diagnosed incidentally during elective endoscopy and review the available literature. Results: This neoplasm consists of glandular proliferation preferentially located in the proximal duodenum. Its diagnosis, normally made by endoscopic biopsy, can be associated with complications that, although infrequent, should not be underestimated. Conclusions: Duodenal neoplasms are a small percentage of those that affect the gastrointestinal tract. Because diagnosis is usually made by chance during a scheduled endoscopy, treatment should be based on the symptoms and size of the lesion according to the treatment standards of each medical center.

ABSTRACT

We report a case of adenocarcinoma originating from the duodenal Brunner glands in a 47-year-old female patient. The lesion was 0.8 cm in extent and located at the posterior wall of the first part of the duodenum. Histologically, the tumor showed transition from non-neoplastic Brunner glands through dysplastic epithelium into adenocarcinoma. The carcinoma cells were strongly positive for MUC6 protein, which is an epithelial marker for the Brunner glands. Tumor protein p53 was overexpressed in the carcinoma cells, but not in the non-neoplastic or dysplastic epithelium. Dystrophic calcification was predominant. This is the first case report of duodenal adenocarcinoma of Brunner gland origin in Korea.

ABSTRACT

BACKGROUND/AIMS: Sporadic non-ampullary duodenal neoplasms are rare and optimal treatment for these lesions remains undefined. Endoscopic resection of duodenal neoplasms is widely used recently and it is an alternative treatment strategy to surgical excision. This study aimed to evaluate the safety and efficacy of endoscopic resection of duodenal neoplasms and to determine its outcomes. METHODS: Patients who underwent endoscopic resection for non-ampullary duodenal neoplasms between January 2005 and December 2014 were analyzed retrospectively. Data including size, morphology, histology, location and endoscopic procedural technique were reviewed. The main outcome measurements were success rate, complication, recurrence and follow-up assessments. RESULTS: The study included 33 patients with duodenal neoplasms. The mean size of resected lesion was 8.58 mm. The results of histologic examination were as follows: 23 (69.7%) adenomas, 2 (6.1%) adenocarcinoma, 3 (9.1%) Brunner's gland tumor and 3 (9.1%) neuroendocrine tumor. Tubular adenoma wase the most common type (63.6%) of non-ampullary duodenal neoplasms. Eighteen (54.5%) lesions were found in the second portion of the duodenum, and 10 (30.3%) lesions on bulb and 3 (9.1%) lesions on superior duodenal angle. Of the 33 cases, 32 (97.0%) were managed by endoscopic mucosal resection technique during a single session and one case was managed by endoscopic submucosal dissection (ESD). One episode of perforation occurred after ESD. During a median follow-up period of 5.76 months, recurrence was observed in only one case of in a patient with tubular adenoma. CONCLUSIONS: Endoscopic resection of duodenal neoplasm is a safe and effective treatment modality that can replace surgical resection in many cases. Careful endoscopic follow-up is essential to manage recurrence or residual lesions.

ABSTRACT

Brunner’s gland hamartomas are small benign lesions that are most commonly found in the bulb of the duodenum. They are very uncommon, and most are found incidentally during upper gastrointestinal series or esophagogastroduodenoscopy. The lesions tend to be asymptomatic, but patients may present with symptoms of duodenal obstruction or hemorrhage secondary to ulceration. Histologically, a Brunner's gland hamartoma consists of the components of Brunner's gland cells, as well as glandular, adipose and muscle cells. In this study, we report the case of a 30-year-old man who presented with upper gastrointestinal bleeding and obstructive symptoms due to a giant Brunner's gland hamartoma in the duodenal bulb. The hamartoma was successfully removed by endoscopic resection. No significant complications were observed. Microscopically, the lesion was found to be entirely composed of variable Brunner's glands and adipocytes.

ABSTRACT

Brunner's gland hyperplasia, also known as Brunner's gland adenoma or hamartoma, is often encountered as a proliferative lesion arising from the Brunner's glands of the duodenum. This lesion has previously been described as being benign, with no malignant potential. We report a case of Brunner's gland hyperplasia that was incidentally discovered on endoscopy for evaluation of gastrointestinal bleeding. The histopathological findings of this lesion revealed a marked dysplastic and malignant changes from the normal Brunner's gland structure. Our case demonstrates the possibility of malignant potential in the natural history of Brunner's gland hyperplasia, even though its incidence is very rare.

La hiperplasia de las glándulas de Brunner constituye una lesión benigna infrecuente localizada en la primera o segunda porción duodenal. El espectro de la misma incluye la hiperplasia nodular circunscrita, la difusa y los adenomas de las glándulas de Brunner. Reportamos dos casos, uno con un adenoma de las glándulas de Brunner como un pólipo duodenal y el otro como una hiperplasia nodular difusa del bulbo duodenal.


Brunner’s gland hyperplasia is an infrequent benign injury located on the first or second portion of the duodenum. The disease spectrum includes diffuse nodular hyperplasia, circumscribed nodular hyperplasia, and Brunner’s gland adenoma. We report two cases, one with an adenoma of Brunner’s glands as a duodenal polyp and the other as a diffuse nodular hyperplasia of the duodenal bulb.

ABSTRACT

Brunner's gland hyperplasia, also known as Brunner's gland adenoma, polypoid hamartoma, or Brunneroma, is a rare proliferative lesion arising from the Brunner's glands of the duodenum. These lesions have been described previously as benign, with no malignant potential. We report a case of Brunner's gland hyperplasia in the duodenum that was discovered incidentally during the endoscopic evaluation of anal bleeding. Histopathological examination of this Brunner's gland hyperplasia revealed well-marked, low-grade dysplasia. This case suggests a dysplastic stage with malignant potential in the natural history of Brunner's gland hyperplasia.

Se reporta el caso de un paciente varón de 69 años con un gran adenoma de las glándulas de Brunner que se presentó con anemia severa sintomática y pérdida de peso significativa. El estudio endoscópico alto detectó una gran tumoración duodenal que se extendía del bulbo hasta la segunda porción y la biopsia fue informada como duodenitis crónica inespecífica con cambios metaplásicos gástricos. El paciente fue sometido a una duodenopancreatectomía y la histología de la pieza quirúrgica fue un tumor de glándulas de Brunner. El adenoma de glándulas de Brunner se considera un tumor benigno del duodeno; sin embargo, la resección endoscópica está limitada a lesiones menores.


We report the case of a 69-year-old male patient with a large Brunner's gland adenoma, who presented with symptomatic anemia and significant weight loss. The upper gastrointestinal endoscopy showed a large duodenal tumor extending form the bulb to the second portion, and the biopsy was reported as chronic non-specific duodenitis with gastric metaplastic changes. The patient underwent a duodenopancreatectomy, and the histological study of the surgical specimen was reported as a Brunner's gland tumor. Brunne's gland adenoma is considered a benign tumor of the duodenum; however, endoscopic resection is restricted to minor injuries.

Cystic Brunner's gland hyperplasia is a benign lesion of the duodenum that may not be familiar to gastroenterologists because of its rarity. Moreover, cystic Brunner's gland hyperplasia with intacystic inverted growth is more uncommon lesion. Here, we report a case of cystic Brunner's gland hyperplasia in a 34-year-old man. An endoscopy of the upper digestive tract revealed the presence of a 1.2 cm sized polypoid mass of the duodenal second portion. Endoscopic submucosal dissection was performed to remove the mass. Microscopically, a cystic lesion was seen in the submucosa underneath the normal surface duodenal mucosa. Hyperplastic Brunner's glands were seen close by the cyst. The cyst was lined by a layer of columnar epithelium similar to that of Brunner's gland ducts with no cytologic atypia. Also, a part of the cyst lining was inverted into the cyst lumen with fibrovascular core. Therefore, The final diagnosis was a cystic Brunner's gland hyperplasia with intracystic inverted growth.

ABSTRACT

Brunner's gland adenoma is a rare benign tumor that is usually asymptomatic and it the result of a benign proliferation of the Brunner's glands of the duodenum. In symptomatic patients, the common clinical presentation is gastrointestinal bleeding and obstructive symptoms. A 48-year-old man presented with abdominal discomfort and vomiting. The endoscopic examination revealed a large pedunculated polypoid mass arising in the bulb and it was prolapsing through the pylorus into the antrum, and this all resulted in a ball-valve obstruction. After endoclips were applied at the peduncle of the mass, this polyp was simply and successfully cut with using an IT-knife without bleeding or perforation. We report here on a case of Brunner's gland adenoma that caused a ball-valve obstruction, and the tumor was removed by a simple and easy method with using endoclips and an IT-knife.

En el presente reporte presentamos a un paciente varón de 84 años con diarrea crónica y anemia megaloblástica; la semana previa a su hospitalización presentó naúsea y vómitos. Se le realizó un examen endoscópico alto que detectó una lesión elevada submucosa -de 40 mm- en el bulbo duodenal, la biopsia no fue contributoria. La diarrea y la anemiamegaloblástica se resolvieron con cianocobalamina y ácido fólico; posteriormente la lesión fue resecada quirúrgicamente. La histología del espécimen quirúrgico correspondió a untumor de las glándulas de Brunner. El tumor de las glándulas de Brunner ûtambién conocido como Brunneroma- es una lesión benigna poco frecuente del duodeno; puede descubrirse incidentalmente en una endoscopía; sin embargo, en algunas ocasiones pueden causar hemorragia u obstrucción. Existe aún controversia respecto a si es un hamartoma o un verdadero adenoma.


We present a 84 year old male with a history of chronic diarrhea and megaloblastic anemia. A week before his hospitalization had nausea and vomit. An upper endoscopy showed asubmucosa elevated lesion of 40 mm in the duodenum bulb, the biopsy was not contributory. The diarrhea and megalobastic anemia resolved with cianocobalamina and folic acid, laterthe submucosal lesion was removed by surgery. The histological study of the specimen reveeled a BrunnerÆs glands tumor. The BrunnerÆs glands tumor, also known as brunneroma, is a rare benign duodenumÆs lesion; it could be incidentally discover during an endoscopy and sometimes it cause of bleeding or obstruction. There is controversy if this tumor is anhamartoma or a true adenoma.

ABSTRACT

Brunner's gland adenoma is a rare tumor of duodenum. Patients are usually aymptomatic and most are discovered incidentally during the upper gastrointestinal (GI) series or esophagogastroduodenoscopy. These lesions are most commonly located in the duodenal bulb and clinical manifestations are variable. In symptomatic patients, the most common manifestations are GI hemorrhage and duodenal obstruction. On histologic examination, Brunner's gland adenoma that causes clinical symptoms is composed of hyperplastic Brunner's glands and contains mostly an admixture of glandular, adipose, and muscular tissues. We report a case of large Brunner's gland adenoma causing upper gastrointestinal hemorrhage in a 47-year-old woman which was successfully removed by endoscopic resection without complications such as bleeding or perforation. Microscopically, it was entirely composed of variable Brunner's glands.

ABSTRACT

Thymoquinone [TQ] was demonstrated as the major active component of Nigella sativa seed oil. It was reported that thymoquinone has an anti-inflammatory and immune-stimulatory effects. On the other hand, cortisone is one of the steroid hormones that used as an anti-inflammatory and immune-suppressor drug. In the present study, the effects of both thymoquinone and cortisone were studied on the histological structure of both duodenum and superior mesenteric lymph nodes of rats. It was noticed that there was an increase of the duodenum mucosal mucus-secreting cells and some nuclear changes in the nuclei of the Brunner's gland cells. There was an increase in number of macrophages, plasma cells and lymphocytes in the superior mesenteric lymph nodes. These findings were discussed according the anti-inflammatory and the immunestimulatory effects of thymoquinone as a point of view, and the anti-inflammatory and immune-suppressing effects of cortisone on another point of view. It was concluded that thymoquinone has an anti-inflammatory and immune sthnulatory effects on rat duodenum and superior mesenteric lymph nodes. However, more investigations are still in need to recommend the replacement of this natural ingredient instead of cortisone with its side effects

ABSTRACT

Brunner's gland hyperplasia is a rare tumor of the duodenum and might also be an unusual cause of gastrointestinal bleeding. In symptomatic patients, treatment requires either surgical resection or endoscopic polypectomy. We report a case of upper gastrointestinal bleeding from a pedunculated Brunner's gland hyperplasia in the duodenal bulb. Endoscopic resection using the detachable snare and hemoclipping was instituted to remove a large pedunculated polyp. The pathologic diagnosis was Brunner's gland hyperplasia with adenomyomatous hyperplasia.

ABSTRACT

Brunnera's gland hyperplasia is a benign tumor of the duodenum and it is rarely associated with clinical symptoms. We report on a 64-yr-old man with Brunnera's gland hyperplasia who had undergone a duodenocephalo-pancreatectomy. The reason is that he presented upper gastrointestinal obstructive symptoms and the esophagogastroduodenoscopic finding revealed the lesion to be an infiltrating type mass on the second portion of the duodenum with luminal narrowing. An abdominal computed tomography showed a 2.5 cm-sized mass in the duodenal second portion with a suspicious pancreatic invasion and 7 mm-sized lymph node around the duodenum. Duodenocephalopancreatectomy was successfully performed. Histological examination revealed a Brunnera's gland hyperplasia. The final diagnosis was the coexistence of Brunnera's gland hyperplasia and pancreatic heterotopia with a pancreatic head invasion. The literature on Brunnera's gland hyperplasia is reviewed.
